What's Inscribed
All VA headstones and markers include:
- Name
- Years of birth and death
- Branch of service
- Military rank
- War service (if applicable)
- Religious emblem (optional, from approved list)
- Military awards (optional)
How to Apply
-
Complete VA Form 40-1330
The Claim for Standard Government Headstone or Marker. Available at VA.gov or through your funeral director.
-
Submit within 2 years of burial
Applications should be submitted as soon as possible after burial, but no later than 2 years after death.
-
Coordinate installation with the cemetery
The VA ships the headstone or marker to the cemetery. The cemetery is responsible for setting it — there may be a fee for this from the private cemetery.
Note: VA headstones and markers are provided at no cost for veterans who died on or after November 1, 1990. For veterans who died before that date in a private cemetery without a government marker, a medallion (attached to a purchased marker) may be available instead.
